ENDOMETRIOSIS
Period pains are common, but if the pain is not relived by "over the counter" painkillers, so bad that you miss school, sport or work, so painful that you have to stay in bed, if they are getting worse month after month and accompanied by stomach pain and back ache, you are maybe suffering from Endometriosis.
What is Endometriosis?
The inside of your uterus (womb) has a lining that contains blood and tissue. This lining comes away when you have your period. Endometriosis occurs when some of this lining is found outside your uterus in other areas of the body such as on ovaries, bladder and bowel. The endometrial tissue responds to the monthly surges of oestrogen by becoming blood filled, and at menstruation, when the uterine endometrium is shed, the endometrial islets also bleed, but this blood has nowhere to go: it thus creates local inflammation, which, in pelvic and abdominal tissues, is very painful. In addition, this condition is often the underlying reason for inability to conceive. As there may be other medical reasons for bad period pains, a medical diagnosis has to be made.
To treat endometriosis, there are three ways:
Surgery:
Removal of the islets of uterine tissue outside the womb: it can often provide relief for many years, but the problem may come back, sadly, many women have to undergo the same difficult process again. Some doctors recommend hysterectomy, a major surgical operation and the end to women’s hopes of having a family.
Hormonal treatment:
The hormonal treatments for endometriosis mimic the state of pregnancy: classical birth control pills postpone ovulation and therefore control the production of oestrogen in the body. Side effects are common: weight gain, intermittent bleeding, mood swings, headaches and no one knows how long it is safe for a woman to be on continuous birth control pills. Some doctors prescribe progesterone derivatives only, because oestrogen stimulates endometriosis.
Other hormonal alternatives are drugs that decrease the amount of oestrogen in the body by controlling the pituary gland and thus forcing the body into artificial menopause. Side effects include hot flushes, vaginal dryness, headaches, depression, insomnia, and memory loss. Treatment is limited to 6 months and the recurrence rate is high.
Testosterone (male hormone) derivatives are other hormonal alternatives, they shrink the endometrial islets and thus reduce pain, but have serious side effects (acne, reduction in breast size, weight gain, abnormal facial and body hair growth, deepening of the voice).
Natural therapies:
Avoiding caffeine, sugar, and alcoholic beverages, making dietary changes, consuming soy proteins, using stress-reducing techniques and acupuncture have all brought some help to sufferers. Evening primrose oil, B complex vitamins (improving emotional symptoms of endometriosis and linked to the breakdown of oestrogen in the body), and vitamin E and Selenium (when taken together decreasing endometriosis-related inflammation) are commonly used.
